Plugin Breaks WordPress Dashboard

All the checkboxes (for example on the “all posts” or “all pages” or “update” listings) disappear and the text turns green when active. When I disable your plugin everything goes back to normal. In the attached image, note the text is green and checkboxes have disappeared.
