Mail Chimp Integration

I recieve the following Error when trying to save the integration on the campaign: Updating the campaign failed with the message: SQLSTATE[HY000]: General error: 1712 Index ndx_om_ruleset_actions_slug1 is corrupted (Connection: optinmonster, SQL: select * from wp_om_ruleset_actions where ( ruleset_id = 1669342 and slug = XGYp8ylQBbY2EW6KD8LP) limit 1) then when I test the popup on the site I get the following error. Oops! There was an error submitting the form: Your contact information could not be processed. Please try again later. The response code returned was 404 with the following error: The requested resource could not be found.
