Help with plugin – urgent

Hi support, We used your plugin on this website: https://evolve.thedm.me/home/respondents/ Click on Join us on first slider. We have the next issues: When we activate option Country code for mobile number, the phone number doesnt work (cant click on next or previous step) How can we create age limit, minimum 18 years old on form? When we use multiple select option on select field, we get few more selections named Premium1@, Premium1@ etc. We didnt added them to form but it has been automatically added. Thanks. If you are utilizing an additional plugin for this feature, I recommend reaching out to their support team for assistance. Create an input field for age. Retrieve the input value and Field Id from the age field, and apply hide, show, enable and disable conditions for the fields you want under Conditions Tab. You may use either the ‘greater than (>)’ or ‘greater than equal (>=)’ operators to make the comparisons.
