Form stopped working

Hello, A website I’m maintaining sellds pet tags with the option for the customer to have their details engraced. For that, the website uses the Calculated field forms. We have creted a form with the available fields, the user fills it in and when they makethe purchase, in the order management (in admin), we see the details the added. the details are shown as meta dara to the product. I’m not the one who created the website, so i don’t now exactly what the previous developer did. Now, I have the following problem – I think it occured after I updated the whole website. The form has disapperaed from the product page. I added it again using some code in functions.php to load the shortcode, but althought the form seems to work, I don’t see the detailin the order managment, nor I receive an email with the form submission. Things I have noticed/know: The add-ons section of the plugin settings is empty. Email delivery is working correctly. I have updated the plugin to the latest version. We are using the free version. No theme files have been modified by the update. Any ideas what might have happened? If your website uses WooCommerce or Easy Digital Downloads (EDD) to sell products, then you were using either the Developer or Platinum distribution of the plugin, since the free version does not include add-ons.
