After going through a tedious setup process twice, discovered that this plugin has EXTREMELY limited options. There is no way to embed a form where you want it via shortcode. Cannot be placed in the middle of the sidebar, or on a signup page. It either has to take over and reformat every single page and post on the website, or it has to replace the sitewide sidebar. Also very limited styling options. You have to select one of the few non-premium form themes and accept however it looks. @sejohnsen1 , we have all those options available. Embed via short-code CHECK. Place in the middle of sidebar CHECK. Place on signup page CHECK. Would you like assistance in setting up? Yes, I would welcome it! I’ve created two forms, neither of which provided a shortcode or any other method to place in my sidebar. Can you tell me how to do that? Can you please create a support ticket with us so we can look into. Please make sure that you have created an embed form. The shortcode is provided for the embed form only in the Publish step. We have support article available on our help portal regarding this as well.

Yes, I would welcome it! I’ve created two forms, neither of which provided a shortcode or any other method to place in my sidebar. Can you tell me how to do that?
