when using ULTIMATE TINY MCE the little icon to place the shortcode in Bainternet Simple TOC gets obliterated. I have to disable the TINY MCE to use it. Not a big deal, I know the shortcode, but many people use this ULTIMATE TINY MCE. http://wordpress.org/plugins/bainternet-simple-toc/ It works fine
